Physiotherapists
Previously Licensed in Alberta
All applicants must provide:
- Registration checklist outlining how you will meet Alberta’s registration requirements.
- Reference from the jurisdiction you are leaving to confirm license status and discipline history.
- Work experience for last five years.
- Evidence of personally holding $5,000,000 liability insurance covering all areas of practice.
- Photo ID such as a current driver's license or passport.
- Application and practice permit fee.
- Complete application form.
Applicants not licensed in Alberta within the past 10 years may also need to provide information about their education and exam status. Contact Physiotherapy Alberta before submitting your application to find out what information is on file.
Members on the general register who cancelled their registration after October 1, 2011, must complete the continuing competence program activities required for the year they were last registered prior to being issued another practice permit.
Processing time
We will acknowledge receipt of your application via email within three working days and give you an electronic link where you can monitor your application status.
