How to File a Complaint

Talk to your physiotherapist (or their supervisor) if you have a concern. Immediate and direct communication can resolve many situations quickly and easily. If your complaint involves sexual impropriety, contact Physiotherapy Alberta's complaints director immediately.

To initiate the formal complaint process, Alberta's Health Professions Act  requires the complaint to be submitted in writing. You may choose to use Physiotherapy Alberta's fillable complaint reporting form.  All written complaints must include:

  • Name, address and daytime telephone number (email address is optional)
  • Physiotherapist’s name
  • Incident date, time and location
  • Specific details and all pertinent supporting information
  • Your signature
